Dear Parent or Carer,
I write to advise you that the Birmingham Community Health Care are offering COVID-19 vaccinations for all students, at this school, on Friday 12th November 2021. Due to the urgency of this vaccination programme, it is crucial that you return the enclosed consent form as a matter of urgency.

The vaccinations need to be pre-ordered for the day. Therefore, the immunisation team will not be able to guarantee the pupils will have a vaccine allocated if they fail to return their consent form ahead of the session. Please could you complete, sign and return the enclosed form to me as soon as possible.
Furthermore, students should continue to test twice-weekly over half-term using “Lateral Flow Tests” and preferably take test the night before they return to school after the Half-Term break. This then maximises the chances of everybody staying safe both within school and the wider community.
